Resides within our service area - Roseburg city limits and Winchester

  • Homebound persons over the age of 60

  • Person Eligible for Medicaid/APD voucher approved by DHS case manager or

  • Spouse or Disabled dependent of eligible consumer

Definition of Homebound

  • Leaving the home takes considerable effort (doesn’t drive or minimal)

  • Leaving home unaccompanied is not normally possible

  • Leaving home is only for medical care or infrequent trips such as a haircut or church attendance

  • Physically or mentally unable to shop or prepare meals

  • Inadequate support system for shopping and meal preparation

  • Unable to tolerate a group situation due to physical or mental disability or substance abuse.
